Appointments

Scheduling

We are excited to see you and your child and we want you to have the best experience possible. Please arrive 15 minutes prior to your appointment. At the bottom of this page you will find helpful forms and directions to our office. We encourage you to fill out any forms before you arrive for your appointment. Please bring your insurance card to each visit. Any co-payments are due the day of the appointment (if applicable).

Cancellation and Late Policies

If you need to cancel an appointment, please give us 24 hours notice. We understand life is busy. It’s better to cancel your appointment rather than not show up for the appointment. If you do not show up for 3 scheduled appointments within a 12-month period you will be asked to find a new pediatrician outside of Bay Clinic Pediatrics.

If you are more than 15 minutes late for your appointment, the appointment will be rescheduled.

Well Child Visits

To keep children healthy, it is important to bring them in for regular checkups and immunizations. Call 541-269-0333 ext 400 to schedule your child’s routine checkup. We recommend calling several weeks in advance so that you may choose a date that best fits your schedule.

Sick Child Visits

We are sorry that your child isn’t feeling well and we want to help. Call our office at 541-269-0333 ext 400 to schedule an appointment.

Pediatric Walk-In Clinic

Located in our Annex building on campus (a free standing building).

Staffed by a Pediatric Provider for newborns - 18 years old:

  • Monday – Thursday until 6pm
  • Friday until 5pm

CHADIS

Bay Clinic Pediatrics is proud to provide CHADIS, a developmental screening questionnaire that you fill out at home prior to your visit. A few days prior to your visit go to: www.chadis.com to complete the questionnaire. The invitations code is 5412690333.

  • Newborn check (usually within a day or two after being discharged from the hospital)
  • 2 weeks old
  • 2 months old
  • 4 months old
  • 6 months old
  • 12 months old
  • 15 months old
  • 18 months old
  • 2 years old
  • 2 ½ years old
  • 3 years old
  • …and then yearly
